The horror is very real, as is her fear and misery. Hooks tear at her face, forcing her mouth open and eyes closed, immobilising her completely as she is subjected to slaps, her cunt and tongue are beaten, chopsticks made into her nasal cavaties, and made orgasms wrack her body. Electrical shocks push her closer to the edge of sanity, as the sheer terror of her situation threatens to overwhelm her. Struggling to cope with sexual pleasure in such an extreme environment is even harder for poor Mei than the pain, a problem Magick solves by stapling her cunt shut when it's no longer of any use to him.
